Qualifications

• Active, unrestricted licensure in the state you will practice
• Active ANCC/AANP certification for NP or NCCPA for PA
• Willingness to obtain DEA licensure where required by state regulations

Responsibilities

• Conduct comprehensive assessments including medical history, vitals, and exams
• Coordinate care with interdisciplinary team
• Provide counseling and education to patients and caregivers
• Document accurately including ICD-10 codes
• Support short-term sub-acute rehabilitation needs
• Engage in advanced illness and care planning conversations
