Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician (NP/PA) to join the Senior Community Care team, delivering integrated care to residents in nursing homes, senior housing, and assisted living facilities.

You will conduct assessments, manage care plans, and collaborate with a multidisciplinary team to improve patient outcomes. A flexible, non‑on‑call schedule and competitive compensation are offered in a supportive work environment.

Qualifications

• Active ANCC or AANP certification for NP track or NCCPA certification for PA track
• State licensure in practicing state (or ability to obtain by start date)
• Active and unrestricted driver's license and access to reliable transportation
• Ability to gain a collaborative practice agreement if required by state law
• Willingness to work in a senior home/assisted living care setting and coordinate with interdisciplinary teams

Responsibilities

• Conduct comprehensive assessments including history, meds reconciliation, vitals and exam
• Perform evidence‑based screenings and point‑of‑care testing as appropriate
• Provide care coordination for long‑term care patients in sub‑acute settings
• Maintain thorough documentation including ICD‑10 codes
• Collaborate with interdisciplinary care teams and support care planning
