Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician (NP/PA) to deliver comprehensive care to high-risk members in nursing homes, senior housing, and assisted living settings. The role emphasizes medication reconciliation, advanced illness planning, and collaboration with interdisciplinary teams.

Candidates must hold active NP/PA certification, state licensure, and prescriptive authority where required. A driver’s license and reliable transportation are expected.

Qualifications

• Active, unrestricted professional licensure in the practicing state.
• NCCPA certification for PAs or ANCC/AANP certification for NPs.
• Current DEA licensure or assuming prescriptive authority per state regulations.
• Evidence of ability to obtain collaborative practice agreement if applicable.
• Proven ability to work in varied care settings and to navigate complex patient needs.

Responsibilities

• Review past medical history, conduct physical exams, and perform medication reconciliation.
• Conduct evidence-based screenings and point-of-care testing as appropriate.
• Support members during sub-acute rehabilitation episodes and ensure accurate documentation (ICD-10).
• Communicate with interdisciplinary care teams and discuss advanced illness planning with patients.
• Provide counseling and education to patients and caregivers regarding care plans.
