UnitedHealth Group's Optum Home & Community Care division is hiring an Advanced Practice Clinician to join the Senior Community Care program, serving high-risk members in nursing homes, senior housing, and home care. Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state (or ability to obtain by start date)
• Active and unrestricted driver’s license with reliable transportation
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to gain a collaborative practice agreement where applicable
• Ability to navigate varied environments to perform duties

Responsibilities

• Conduct comprehensive assessments including history, medication reconciliation, vitals and physical exams
• Support members during a short-term sub-acute rehabilitation episode
• Ensure accurate documentation including ICD-10
• Collaborate with interdisciplinary care team
• Conduct advanced illness and advance care planning conversations
• Provide counseling and education to patients and caregivers
