Job summary

Telecare Corporation is seeking a Case Manager II to provide primary support and case management for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their chosen community.

Under supervision, you will coordinate services, arrange transportation, and collaborate with partners to connect clients with resources, programs, and ongoing support within a trauma-informed, compassionate framework.

Qualifications

• High School Graduation or GED equivalent with 2 years direct mental health/community services experience, or an Associate's with 1 year, or a Bachelor's in Social Services.
• Valid and current driver's license; personal vehicle insurance; name listed as driver.
• Willingness to use personal and company vehicles to transport clients to appointments and activities.

Responsibilities

• Under supervision, serve as primary support and case manager for individuals with mental illness and co-occurring disorders to promote independent living in the community.
• Coordinate services, assist with transportation, and collaborate with partners to connect clients with resources and support.
