Job summary

Crisp Regional Hospital, Inc. is seeking a Lead RN MDS Coordinator for full-time daytime hours in Cordele, GA.

The role focuses on coordinating MDS assessments, ensuring regulatory compliance, and supporting care planning for residents in a long-term care setting. Under the Administrator, Nursing Home leadership, the MDS Coordinator completes and submits MDS documentation, participates in care conferences, and collaborates with the interdisciplinary team to maintain high standards of resident

Qualifications

• Requires Nursing degree from an accredited school of nursing.
• Requires a level of experience normally acquired with a minimum of two to four years of progressively more responsible work experience within a long-term care environment or any equivalent combination of education, training and experience.
• Maintains a current state licenses as a Registered Nurse and is listed in good standing with the state’s Department of Professional Credentialing.

Responsibilities

• Ensures that resident care plans are in compliance with established MDS regulations.
• Completes and submits all the MDS documentation on time and according to established schedules.
• Acts as an active member of the interdisciplinary and management team providing expertise related to all current and pending MDS regulations.
• Participates in the planning and adjustment of Medicare assessment scheduling.
• Ensures that Medicare certifications/re-certifications are completed timely and returned to medical records.
• Supplies PPS information to biller and other departments.
• Serves as the Coordinator of care conferences.
• Ensures that all submissions are accurate and compliant with current regulations.
• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents in planning delivery of care.
• Maintains comprehensive and up-to-date knowledge of the MDS rules and regulations as described in the MDS User's Manual.
• Committed to an interdisciplinary treatment approach to rehabilitation by exercising sound nursing judgment.
• Monitors and ensures compliance and consistent application of the Code of Conduct and Compliance Program.
• Develops strong relationships with physicians, administrative team, subordinates and other key stakeholders.
• Performs other related job duties as assigned.
