Lake City Center for Nursing and Healing in Georgia is seeking an experienced MDS Coordinator to ensure accurate resident assessments, high-quality care, and compliance with CMS and state guidelines. You will work in a supportive, team-driven environment and play a key role in care planning and performance improvement.

Qualifications

• Current RN license (Georgia).
• Prior MDS/RAI experience in long-term care preferred.
• Strong organizational and communication skills.
• Knowledge of CMS guidelines and care planning.
• Long term care experience.

Responsibilities

• Coordinate and oversee all MDS (Minimum Data Set) assessments in accordance with CMS and LTCI guidelines.
• Collaborate with nurses, therapists, and department heads to ensure accurate, timely documentation.
• Prepare and analyze Quality Measure/Quality Indicator (QM/QI) data for performance improvement.
• Ensure compliance with all RAI and MDS procedures and submission deadlines.
