Job summary

Topaz Fiscal Services LLC is seeking an MDS Coordinator in Georgia to manage the RAI/MDS process, lead the interdisciplinary care plan, and coordinate PPS/Medicaid case mix meetings. You will schedule assessments, monitor resident status, and ensure accurate documentation in accordance with federal and state regulations.

The role requires RN or LVN licensure, 2 years in long-term care, and experience with RAI/MDS.

Qualifications

• Current active RN or LVN license in the state.
• Registered Nurse license preferred.
• 2 years’ experience in long-term care preferred.
• 6 months in rehabilitative/restorative nursing preferred.
• 2 years’ experience in RAI process completion preferred.

Responsibilities

• Coordinate RAI/ MDS process, ensure timely completion.
• Schedule assessments (OBRA, Medicare PPS, Medicaid) and communicate with departments/families.
• Monitor resident condition and coordinate MDS re-assessments.
• Set ARD with interdisciplinary team to reflect resident needs.
• Complete MDS and CAAs accurately per policies.
