Job summary

The Cincinnati Skilled Nursing Facility is seeking an experienced MDS Coordinator to join our long-term care team in Cincinnati, OH. You will coordinate and complete MDS assessments, ensure timely MDS, CAAs, and care plans, and work with nursing, therapy, social services, and the interdisciplinary team to support quality outcomes.

• Previous MDS Coordinator experience in skilled nursing or long-term care required.
• Working knowledge of Medicare, Medicaid, PDPM, and reimbursement processes.
• Experience with PointClickCare (PCC) preferred.
• RAC-CT certification is a plus.
• Strong organizational, communication, and time management skills.
