Job summary

Fenway Health seeks a Manager of Integrated Behavioral Health and Case Management Services to oversee integrated behavioral health programming within primary care and lead the organization-wide case management initiative.

The role includes direct supervision of CHWs/Care Navigators/Care Coordinators, development of education and training programs, and collaboration with nursing, primary care, Behavioral Health, and ACOs.

Qualifications

• Licensed clinician with LICSW/LMHC/LMFT credentials.
• Minimum 7 years of relevant clinical experience; 4+ years post-licensure in direct client care.
• Minimum 2 years of clinical and administrative supervisory experience.

Responsibilities

• Provide direct patient care for a portion of time each week.
• Oversee clinical and administrative supervision of BH integration staff.
• Oversee CHW/Care Navigator/Care Coordinator team operations and development.
• Lead organization-wide case management program design and implementation.
• Develop population health initiatives and BH education/training programs.
• Participate in quality improvement and interdepartmental initiatives.
