Job summary

Fenway Health seeks a Manager of Integrated Behavioral Health and Case Management Services to oversee integrated behavioral health programming in collaboration with nursing, primary care, and ACOs. The role includes supervising CHWs and leading organization-wide case management, while maintaining a clinical practice and dedicated patient care hours weekly.

The position emphasizes developing education programs, coordinating Ryan White reporting, and driving quality improvements across departments

Qualifications

• Licensed clinician (LICSW/LMHC/LMFT) with active license.
• Minimum 7 years of relevant clinical experience; at least 4 years post-licensure.
• Minimum 2 years of clinical and administrative supervisory experience.
• Minimum 2 years of experience with integrated behavioral health programming.
• Experience supervising community health workers, care navigators, or care coordinators preferred.

Responsibilities

• Provide direct patient care for one day per week as needed.
• Oversee behavioral health integration within primary care teams.
• Supervise CHWs/Care Navigators/Care Coordinators and oversee their operations.
• Lead organization-wide case management program and align services across departments.
• Develop population health initiatives and measure outcomes.
