Centene Corporation is seeking a care management professional to assess and coordinate services for complex populations in a hybrid, in-home setting across Kingsville or Victoria, TX. The role emphasizes developing personalized care plans and educating members and families on available benefits.

The ideal candidate will be a licensed nurse with 4–6 years of related experience, capable of collaborating with providers and communities to ensure high-quality, cost-effective care.

Formación

• Graduate from an Accredited School of Nursing or a Bachelor’s degree and 4–6 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Resource Utilization Group (RUG) certification must be obtained within 90 days of hire

Responsabilidades

• Assesses and coordinates care across the continuum for complex/high acuity populations
• Develops personalized care/service plans for long-term care members
• Educates members and caregivers on services and benefits
• Coordinates with care providers and community resources to meet member needs
• Monitors progress and revises care plans as needed to meet goals
