Centene Corp. seeks a skilled care manager to assess and coordinate services for high-acuity members in a hybrid in-home setting across Kingsville or Victoria.

You will develop personalized care plans, educate families, and collaborate with providers to ensure high-quality, cost-effective care. Responsibilities include monitoring progress, adjusting plans, identifying needed resources, and guiding new hires during onboarding.

Qualificações

• Graduate from an Accredited School of Nursing or a Bachelor's degree with 4–6 years of related experience.
• Bachelor's degree in Nursing preferred.
• RN - Registered Nurse license or NP - Nurse Practitioner license required.

Responsabilidades

• Performs care management duties to assess and coordinate services across the continuum of care.
• Develops personalized care plans for long-term care members and educates families/caregivers.
• Coordinates with care providers and community resources to address member needs.
• Monitors and revises care plans to meet member goals and changes in condition.
• Acts as liaison between member/family and physicians or facilities.
