Job summary

Centene Corporation seeks a qualified Care Manager to perform in-home assessments in the Abilene, TX service area. This hybrid role involves coordinating care plans for complex/high-acuity members with primary medical/physical health needs, educating families, and liaising with providers to ensure high-quality, cost-effective care.

The role requires RN or NP licensure with stateナ licensure, a relevant nursing degree, and 4–6 years of related experience.

Qualifications

• Requires RN or NP licensure with active state licensure.
• Graduate from an accredited nursing program or BSN with 4–6 years of related experience.
• Utilization Group (RUG) certification must be obtained within 90 days of hire.

Responsibilities

• Evaluates service needs of high-risk members and develops a care plan.
• Develops and assesses long-term care plans with care management team.
• Coordinates between members/families and care providers for person-centered care.
• Monitors progress and revises care plans to meet member goals.
• Reviews data to identify trends and improve quality and compliance.
