Job summary

Centene Corp. is seeking a Nurse or Nurse Practitioner to perform care management duties, assess complex/high-acuity members, and coordinate services across the care continuum in Abilene, TX.

This hybrid role includes in-home assessments and collaboration with a multidisciplinary team. The position requires a nursing degree and 4–6 years of related experience, with licensure as RN or NP and willingness to support personalized care plans and family education.

Qualifications

• Responsible for care management duties and coordinating services across the continuum.
• Educates members and families on services and benefit options.
• Supports long-term care planning and member advocacy.

Responsibilities

• Assess and coordinate care for high acuity members.
• Develop and monitor personalized care/service plans.
• Collaborate with providers to address member needs and revise plans as needed.
• Maintain all member information per guidelines and regulations.
