Job summary

Centene Corporation is seeking a RN care coordinator for a hybrid role covering Hidalgo County, TX and surrounding communities. The position involves home visits and remote work with a schedule of Monday–Friday, 8:00 AM–5:00 PM CST.

4–6 years of related nursing/care coordination experience is required, with a BSN preferred. Travel is required for member home visits within Hidalgo County and nearby areas, with mileage reimbursement provided.

Qualifications

• Graduate from an Accredited School of Nursing.
• Requires 4–6 years of related clinical nursing and/or case management and care coordination experience.
• Bachelor of Nursing degree is preferred.
• RN and/or NP licensure required (RN required; NP licensure for Nurse Practitioner).
• RUG certification must be obtained within 90 days of hire (Superior Health Plan).

Responsibilities

• Evaluates the service needs of the most complex/high risk members and develops a care plan.
• Develops and updates long-term care plans and coordinates with providers and community resources.
• Coordinates between member/family and care provider team to ensure appropriate, person-centered care.
