Job summary

Health & Human Services Commission is seeking an Investigator V to lead on-site inspections of Texas long-term care facilities, ensuring compliance with state and federal rules. The role requires extensive travel, on-report writing, and collaboration with facility staff to resolve deficiencies.

Under supervision, you will conduct licensure inspections, respond to incidents, and prepare comprehensive surveys.

Responsibilities

• Conduct on-site licensure inspections and investigations in Texas long-term care facilities.
• Prepare and finalize on-site inspection reports with findings and recommendations.
• Communicate findings clearly with facility staff and stakeholders.
• Travel up to 80% across the state to perform inspections and follow-ups.
• Attend hearings or provide testimony as needed.
