Job summary

Qualifications

Responsibilities

• Supports and promotes the mission and values of Covenant Health Ministry.
• Practices according to the ANA guidelines of care as adopted by the Nursing Division.
• Adheres to the Nurse Practice Act of NH.
• Performs various patient care activities and related services necessary in caring for the personal needs and comfort of patients.
• Documents all care provided to the patient in the Electronic Health Record, including Vital Signs, Intake/Output, Mobility, pain, and skin abnormalities, and related interactions.
• Reports any unusual or changes in patient condition to the primary RN and/or charge RN.
• Completes bedside shift hand-off report routinely and independently.
• Answers call lights to determine patients’ needs and supports timely response times.
• Performs bladder scans, EKGs, CPMs, assists with cold therapy and other appropriate medical procedures after necessary competencies.
• Bathes patients daily and assists with personal hygiene to increase comfort and wellbeing.
• Encourages and assists patients with routine ambulation and mobility and documents activity in the EHR.
• Serves and collects food trays, assists with intake for patients and provides between-meal nourishment.
• Collects samples for testing (urine, fecal occult).
• Assists with specialty beds and mattresses; skin and wound care under nurse direction.
• Expresses interest in patient progress and ensures patient needs are met.
• Uses judgment in providing information and maintains sensitivity to individual and organizational concerns.
• Demonstrates effective verbal and non-verbal communication; provides a secure environment for behavioral health patients after completing competencies.
• Performs continuous visual monitoring as directed by RN and completes related competencies.
• Explains nursing care to involve the patient in their care.
• Rotates among shifts/departments to meet care needs.
• Assists with Hospital Unit Coordinator duties as needed.
• Prepares equipment and assists staff during treatments and examinations.
• Follows safety protocols and transport procedures for patients.
• Understands fall prevention for high-fall-risk patients and reports concerns to RN.
• Uses AIDET with patients and conducts hourly rounds.
• Completes annual competencies by due dates and maintains licensure.
• Maintains adequate patient care hours for licensure.
• Maintains current license and certifications.
• Avoids unauthorized overtime and manages earned time off per policy.
