Job summary

You will plan and coordinate services to address emotional and social needs, while ensuring quality care and compliance with regulations.

Qualifications include a Bachelor’s Degree in Social Work or equivalent, state licensure, and at least two years in long-term care.

Qualifications

• Must be currently registered/licensed in the State.
• Two years of experience in Long Term Care preferred.

Responsibilities

• Plan, organize and direct the Social Services department to meet residents' emotional and social needs.
• Maintain quality of social services provided by the facility.
• Assist in admission, discharge and coordination tasks related to social services.
• Refer patients and families to appropriate community resources.
