Job summary

Kavod Senior Life in Denver, CO, is seeking a Full-Time Assisted Living Care Coordinator to support behavioral health and psychosocial well-being of residents. You will assess needs, coordinate care plans, and connect residents with housing, benefits, and community resources.

This role requires strong collaboration with families, staff, and partners. Experience with aging populations, dementia, depression, and crisis intervention is valued.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field is required.
• Master's degree in Social Work or related behavioral health field preferred.
• Experience working with older adults/aging populations required.
• Strong understanding of psychosocial and behavioral health needs of older adults.

Responsibilities

• Provide short-term behavioral health, psychosocial assessment, advocacy, and supportive services to residents and families.
• Coordinate and facilitate care plan reviews with Wellness Coordinator.
• Collaborate with residents, families, healthcare providers, case managers, staff, and interdisciplinary teams.
• Assist residents with transitions related to housing, healthcare needs, cognitive decline, and loss of independence.
• Coordinate referrals to behavioral health, counseling, community support, housing, and other services.
