Qualifications

• Minimum bachelor’s degree in gerontology, social work or related field.
• Three years of experience providing social services for the elderly.
• Strong written and verbal communication skills.
• Knowledge of MS Office and Excel required.
• Master’s degree and/or certification preferred but not required.

Responsibilities

• Develop relationships with agencies and nonprofits to assist residents.
• Coordinate educational programming, wellness presentations and on-site events.
• Serve as liaison with medical providers and community agencies.
• Report abuse, neglect, or exploitation in compliance with state law.
• Maintain resident files in HUD compliance and prepare reports.
• Create resource directory and assist with transitions into/out of the facility.
• Attend home visits and support move-in processes for prospective tenants.
