Job summary

Silver Tree Residential, LLC is seeking a dependable and experienced Service Coordinator for Greeley Manor, a senior apartment community. The role focuses on connecting residents with community services, coordinating educational programming, wellness events, and on-site activities to promote independence and quality of life.

The ideal candidate will have a bachelor’s degree in gerontology, social work or related field, plus at least three years of social services experience.

Qualifications

• Bachelor’s degree in gerontology, social work or related field.
• Three years of experience providing social services to seniors.
• Master’s degree and/or AASC Professional Services Coordinator certification is preferred but not required.
• Strong written and verbal communication, advocacy and problem-solving for vulnerable adults.

Responsibilities

• Develop relationships with agencies and nonprofits to provide resident services.
• Organize educational programming, wellness presentations and on-site events.
• Serve as liaison with medical, professional services, and community providers.
• Report abuse, neglect or exploitation in compliance with state statutes.
• Maintain resident files per HUD and STR requirements and prepare regulatory reports.
• Create and maintain a resource directory for the community.
• Assist residents with transition into and out of the facility as consumer-directed.
• Attend inspections with Community Director and coordinate plans to age in place.
• Create monthly calendar and site newsletter with input from staff.
• Attend home visits to discuss the Service Coordination program and move-in process.
