Job summary

Someren Glen in Centennial, CO is seeking a resident services professional to inform residents and families about health and support options, develop community referrals, and educate on available services to ensure a continuum of care.

The role involves reviewing resident eligibility, monitoring service delivery, basic case management, maintaining files, and collaborating with staff and community organizations to support well-being.

Qualifications

• Experience in social work, healthcare or gerontology.
• 2+ years direct service with older adults, preferably in assisted living or independent living.
• Crisis response skills.
• Excellent customer service.
• English language proficiency (read, write, speak).

Responsibilities

• Inform residents and families about available health and support services.
• Develop contacts with service providers for resident referrals.
• Educate residents on services and monitor provisions.
• Foster a continuum of care for residents.
• Review residents for admittance and ongoing residence eligibility.
• Monitor delivery of services for timeliness and quality.
• Provide limited case management when needed.
• Maintain resident files and document pertinent information.
• Assist management in identifying residents needing help.
• Support residents as active community members.
• Other duties as assigned.
