Qualifications

• High school diploma or GED.
• 3+ years of experience in services for older adults, people with disabilities, or social services in a healthcare setting.
• Preferred: Bachelor's degree and 5+ years in the above.

Responsibilities

• Coordinate connections to supportive services and transitions to higher levels of care as needed.
• Check in regularly with vulnerable residents and follow up after hospital stays or therapy.
• Build a local resource guide and prepare HUD-related monthly and annual reports.
• Create newsletters, update the website, and serve as the contact for resident groups.
• Plan events, classes, and programs that keep residents engaged and connected.
• Work with management to ensure lease and HUD compliance.
