Job summary

Jaybird Senior Living is seeking a Healthcare Coordinator (RN) to partner with the Community Director in delivering exceptional, person-centered care within a senior living setting. You will lead the resident care team, oversee care programs, ensure regulatory compliance, and uphold HIPAA guidelines.

Responsibilities include assessing resident needs, coordinating care, managing budgets and inventories, and providing 24/7 on-call support for emergencies.

Qualifications

• RN with current state licensure.
• Current CPR certification.
• 4+ years healthcare management experience in senior living or long-term care.
• Minimum 2 years’ experience supporting residents with dementia.
• Strong leadership, organizational and interpersonal skills.
• Ability to work flexible schedules, including weekends and overnight coverage.

Responsibilities

• Assess residents’ needs and create individualized service plans.
• Coordinate all aspects of resident care including EHR/eMAR documentation.
• Monitor medication management and perform regular reviews.
• Communicate with physicians, families and staff regarding health concerns.
• Recruit, hire, train, supervise and evaluate Resident Assistants and healthcare staff.
• Lead onboarding and ongoing training for staff.
• Ensure regulatory compliance and HIPAA adherence.
