Jaybird Senior Living in Minnesota is seeking a Healthcare Coordinator (RN) to partner with the Community Director to ensure exceptional, people-centered care in a senior living setting. The role leads and develops the resident care team and oversees care programs.

Responsibilities include regulatory compliance, HIPAA, coordination of care, EHR/eMAR documentation, budget and staffing management, 24/7 on-call support, and staff onboarding and training.

Qualifications

• RN with current state licensure.
• Current CPR certification.
• 4+ years healthcare management experience in senior living or long-term care.
• Minimum 2 years supporting residents with dementia.
• Strong leadership and interpersonal skills.
• Ability to work flexible schedules including weekends and overnight coverage.

Responsabilités

• Assess residents’ needs and create individualized service plans.
• Coordinate all aspects of resident care, including EHR and eMAR documentation.
• Monitor medication management and complete regular reviews.
• Communicate proactively with physicians, families, and staff regarding health concerns.
• Recruit, hire, train, supervise, and evaluate resident assistants and healthcare staff.
• Lead new hire onboarding, staff in-services, and ongoing training.
• Ensure regulatory compliance, safety, and proper use of protective equipment.
• Review, document, and follow up on incident reports.
• Manage healthcare department budgets, staffing schedules, and medical inventory.
• Provide 24/7 on-call support for resident emergencies and community needs.
