Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for a full-time role delivering senior living visits in Central Bucks County, PA. The position includes a $10,000 sign-on bonus and extensive patient care in home settings.

The PT will perform assessments, develop plans of care, and collaborate with nursing and other professionals to support safe, independent living for adult and geriatric clients. Strong EMR skills and a PA PT license are required.

Qualifications

• A PA Physical Therapist (PT) license.
• At least 1 year of PT experience.
• Graduate from an APTA-approved program or AMA ACCE-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, transfer status, balance, strength, endurance, and safety assessments.
• Develop and implement individual care plans with manual therapy, gait training, balance and other interventions.
• Educate clients and families on rehabilitative care to promote health and independence.
• Documentation of observations and interventions in EMR.
