BAYADA Home Health Care in Philadelphia is seeking an experienced Physical Therapist (PT) for full-time or part-time home health visits to adult and geriatric clients in Northeast Philadelphia. The role emphasizes independent practice, patient education, and collaborative multidisciplinary care to promote health and independence.

Formación

• Current PA Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from a program approved by the APTA or AMA Committee as indicated by transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, transfer status, gait, balance, and endurance.
• Implement wound care interventions with evidence-based dressings and topical agents.
• Develop and implement individualized care plans including manual therapies, gait training, balance, and related interventions.
• Continuously assess and revise the PT care plan and collaborate with nursing in the multidisciplinary plan.
• Educate clients, families, or caregivers in rehabilitative care to promote health, safety, and independence.
• Accurately document observations and interventions using a state-of-the-art touch pad tablet.
