Job summary

Ultra Health Providers is seeking a Nurse Practitioner to deliver onsite medical, mental, and behavioral health services within skilled nursing and senior living communities. You will work independently, manage complex cases, and build trusted clinical relationships with residents in a supportive interdisciplinary team.

Qualifications

• Active, unrestricted NP license in the applicable state(s).
• Current national NP certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct initial admissions and follow-up assessments, including physical exams, medication review, and screenings.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate clearly with residents and families regarding diagnoses, treatment plans, and changes in condition.
