Job summary

Ultra Health Providers delivers onsite medical, mental, and behavioral health services within senior living environments. As a Nurse Practitioner, you will be the consistent clinical presence, providing proactive, patient-centered care and collaborating with an interdisciplinary team to ensure continuity and quality for residents.

You will manage complex conditions independently, build trusted relationships, and contribute to reducing hospital readmissions while working in supportive facilities.

Qualifications

• Active, unrestricted Nurse Practitioner license in the United States.
• Current national Nurse Practitioner certification (ANCC/AANP or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and related settings.
• Independently evaluate, diagnose, and treat acute and chronic conditions.
• Conduct initial admissions and follow-up assessments with appropriate exams and reviews.
• Monitor resident condition changes and adjust treatment plans accordingly.
• Order, interpret diagnostic tests and prescribe therapies within scope.
• Collaborate with Medical Directors, attending physicians, and nursing leadership.
• Communicate clearly with residents and families about diagnoses and plans.
• Maintain accurate documentation in the EMR per regulations.
• Contribute to quality and care coordination to improve outcomes.
• Uphold HIPAA and regulatory standards in practice.
