Qualifications

• Must hold valid NP or PA license in state of operations.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.

Responsibilities

• Provide direct patient care and ongoing management in senior living facilities.
• Perform comprehensive assessments and develop treatment plans.
• Conduct rounds, adjust plans, and coordinate care with other providers.
• Educate residents and families on wellness and disease prevention.
• Maintain EMR documentation and ensure regulatory compliance.
• Participate in quality improvement to enhance care and outcomes.
