Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to provide high-quality primary care in skilled nursing and senior living facilities. You will focus on wellness, chronic disease management, and care coordination to improve seniors’ health and quality of life.

The role requires travel to multiple facilities and offers opportunities to impact care delivery, supported by a modern tech platform and CME resources.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations with current board certification
• Masters Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills preferred
• Excellent written and oral communication skills
• Punctuality, organizational skills, and ability to function within a team

Responsibilities

• Perform comprehensive assessments and develop treatment plans for residents
• Conduct regular rounds to assess conditions and adjust plans
• Coordinate care with staff, families, and other providers
• Educate residents and families on disease prevention and wellness
• Maintain EMR documentation with regulatory compliance
• Participate in quality improvement to enhance care delivery
