Job summary

Qualifications
- Valid NP or PA license in state of operations and current board certification.
- Master's degree in nursing or PA studies.
- Strong leadership, interpersonal, and problem-solving skills.
- Excellent written and oral communication skills.
- Willingness to travel to assigned facilities.
Responsibilities
- Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents.
- Conduct regular rounds to assess residents’ conditions and adjust treatment plans.
- Coordinate care with facility staff, family members, and other providers.
- Educate residents and families on disease prevention and wellness.
- Maintain accurate EMR documentation and ensure regulatory compliance.
- Participate in quality improvement initiatives to enhance care delivery.
