Job summary

The position requires an LPN license, 3–5 years healthcare experience (long-term care preferred), and knowledge of Medicare reimbursement and RUGs. Join our mission-driven team focused on resident-centered care and compliance.

Qualifications

• Must be a Graduate of an accredited school of nursing.
• Experience in MDS or long-term care is preferred.
• Strong attention to detail and analytical thinking required.
• Excellent communication and interdisciplinary collaboration skills.

Responsibilities

• Coordinate and complete MDS assessments per federal and state guidelines.
• Ensure admissions assessments, nursing notes, care plans, and documentation are accurate and current per standards.
• Complete assigned MDS sections and required assessments (skin, falls, incontinence, catheters, etc.).
• Review and update resident care plans based on assessment findings.
• Participate in Medicare meetings, care planning, and IDT discussions.
• Support survey readiness and ensure regulatory compliance across the facility.
