Americare Senior Living in Sabetha, KS is seeking an MDS Coordinator to lead accurate assessments, regulatory compliance, and quality resident outcomes in a skilled nursing community.

We are looking for a clinically skilled RN with MDS 3.0 experience to coordinate MDS processes, collaborate with the interdisciplinary team, and support survey readiness while ensuring timely, compliant documentation.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license.
• Experience with MDS 3.0 and skilled nursing.
• Strong attention to detail, organization, and analytical skills.
• Ability to collaborate effectively with interdisciplinary teams.

Responsabilités

• Coordinate and complete MDS assessments in accordance with federal and state guidelines.
• Ensure admission assessments, nursing notes, care plans, and required clinical documentation are accurate and current.
• Complete assigned MDS sections, assessments, and RAP summaries.
• Review, update, and maintain resident care plans based on assessment findings.
• Participate in Medicare meetings, care plan meetings, and interdisciplinary team discussions.
• Assist in maintaining compliance with state, federal, and Americare standards.
• Support survey readiness and participate in inspections as needed.
• Communicate changes in resident condition to the Administrator and nursing leadership.
• Assess nursing supply and equipment needs.
• Provide on-call support to staff as needed.
