Job summary

Americare Senior Living is seeking a detail-oriented MDS Coordinator to support accurate assessments, regulatory compliance, and quality resident outcomes within our skilled nursing community.

As a licensed RN with MDS 3.0 experience, you will collaborate with nursing leadership and interdisciplinary teams to ensure timely documentation, care planning, and Medicare processes, while maintaining survey readiness and high-quality patient safety.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license required.
• Experience with MDS 3.0 in skilled nursing.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state guidelines.
• Ensure admission assessments, nursing notes, care plans, and required clinical documentation are accurate and current.
• Complete assigned MDS sections, assessments, and RAP summaries.
• Review, update, and maintain resident care plans based on assessment findings.
• Participate in Medicare meetings, care plan meetings, and interdisciplinary team discussions.
• Assist in maintaining compliance with state, federal, and Americare standards.
• Support survey readiness and participate in inspections as needed.
• Communicate changes in resident condition to the Administrator and nursing leadership.
• Assess nursing supply and equipment needs.
• Provide on-call support to staff as needed.
