Americare Senior Living in Sabetha, KS is seeking a detail-oriented MDS Coordinator to support accurate assessments, regulatory compliance, and quality resident outcomes in our skilled nursing community.

The role requires a graduate of an accredited nursing program, current RN license, and experience with MDS 3.0. You will collaborate with nursing leadership and interdisciplinary teams to ensure timely, compliant documentation and care planning.

Qualifications

Graduate of an accredited school of nursing.
Current RN license.
Experience with MDS 3.0 in a skilled-nursing setting.
Strong attention to detail and analytical skills.
Ability to collaborate with interdisciplinary teams.

Responsibilities

Coordinate and complete MDS assessments in accordance with federal and state guidelines.
Ensure admission assessments, nursing notes, care plans, and required clinical documentation are accurate and current.
Complete assigned MDS sections, assessments, and RAP summaries.
Review, update, and maintain resident care plans based on assessment findings.
Participate in Medicare meetings, care plan meetings, and interdisciplinary team discussions.
Assist in maintaining compliance with state, federal, and Americare standards.
Support survey readiness and participate in inspections as needed.
Communicate changes in resident condition to the Administrator and nursing leadership.
Assess nursing supply and equipment needs.
Provide on-call support to staff as needed.
