Job summary

Carillon Senior Living in Lubbock, TX is seeking an experienced MDS Coordinator to join our Health Center team. The role supervises MDS coordinators, ensures accurate and timely MDS and care plans, and coordinates with physicians and family.

You will manage the MDS process per state and Medicare standards and support uninterrupted resident care with strong teamwork. The ideal candidate holds a nursing degree, an active Texas license, CPR certification, and proven experience with MDS software and

Qualifications

• Graduate of an accredited school of professional nursing.
• Current Texas State Board nursing license.
• Current CPR (Healthcare Provider) card.
• Experience with MDS transmittal process and regulations.
• In-depth experience with MDS computer software.
• Expertise in MDS processes and systems.
• Effective oral and written communication skills.
• Team player; cooperates with other departments.

Responsibilities

• Supervises subordinate MDS coordinators.
• Ensures accuracy and timeliness of MDS and care plans.
• Oversees monthly Triple Check meetings.
• Coordinates MDS, resident care plans and conferences on the computerized system.
• Ensures MDS submissions meet HFCA and Medicare standards.
• Participates in family/resident care plan conferences.
• Maintains professional communication across shifts and departments.
