Qualifications

• Current Nursing License (RN) in good standing.
• Experience in geriatric care and resident-centered planning.
• Strong assessment, documentation, and communication skills.
• Familiarity with nursing home regulations and compliance requirements.

Responsibilities

• Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments.
• Ensure timely communication with nursing and other departments for accurate MDS data.
• Review MDS and care plan files for accuracy and completeness.
• Maintain proficiency in MDS software and data entry for reporting.
• Attend and coordinate interdisciplinary meetings for care planning.
• Update and revise care plans based on current assessments.
• Generate Quality Indicators Reports for leadership review.
• Submit MDS data per reporting standards.
• Stay current with industry standards through professional development.
• Coordinate ancillary data reporting to corporate billing.
