Job summary

Bonaventure Senior Living's Spring Creek community is seeking a compassionate Health and Wellness Coordinator to support the health services department, onboard new residents, coordinate care, and help ensure high-quality health care.

The ideal candidate will supervise and train staff, balance regulatory responsibilities with resident care, and communicate with families. CPR is preferred; background checks and drug testing are required; English proficiency is essential.

Qualifications

• Experience supervising and training staff.
• Proficiency in English is required.
• Knowledge of state regulations and company policies.
• CPR certification is preferred.
• Must pass a criminal background check and drug test.

Responsibilities

• Welcome new residents and help provide a smooth onboarding experience.
• Participate in daily stand-up meetings and communicate resident concerns to leadership.
• Conduct regular quality assurance, operational, and resident-chart audits.
• Review reports, coordinate interventions, and assist with regulatory reporting.
• Maintain health services documentation, including state assessment tools and service plans.
• Coordinate care plans, medications, nursing assessments, and changes in condition.
• Manage care coordination with outside providers and keep families informed.
• Lead health services training and track staff credentials and testing.
