Job summary

Bonaventure Senior Living in Oregon is seeking a compassionate Health and Wellness Coordinator to support the daily operations of our health services department. In this role you will assist with onboarding residents, coordinate care, and ensure consistent, high-quality health care.

The ideal candidate is a strong communicator and dependable leader who can balance regulatory responsibilities with genuine care for residents and families, while maintaining accurate documentation.

Qualifications

• State certifications and licenses required or to be obtained.
• Experience supervising team members and providing training and support.
• Understand and follow state regulations and company policies.

Responsibilities

• Welcome new residents and support onboarding.
• Participate in daily stand-up meetings and communicate resident concerns.
• Perform quality assurance, operational, and resident-chart audits for Assisted Living and Memory Care.
• Review incident reports and coordinate interventions with regulatory reporting.
• Maintain health services documentation including assessments, service plans, progress notes, and records.
• Coordinate service plan updates, provider recommendations, medication revisions, nursing assessments, and changes in condition.
• Manage care coordination with outside providers, admissions, plans, and documentation.
• Maintain regular communication with residents' families and facilitate family meetings.
• Communicate dietary needs and preferences to the team and keep records current.
• Lead Health Services training and track staff credentials and testing; present wellness education.
• Participate in resident retention efforts and after-hours schedules.

What Will You Be Doing?

The health and wellness coordinator plays an important role in keeping our health services department organized, compliant, and focused on residents' well-being. In this role, you will be responsible for the following tasks:

• Welcome new residents and help provide a smooth, supportive onboarding experience.
• Participate in daily stand-up meetings and communicate resident concerns, incidents, and other operational issues with community leadership.
• Complete regular quality assurance, operational, and resident-chart audits for Assisted Living and Memory Care.
• Review occurrence and incident reports, coordinate appropriate interventions, and assist with required regulatory reporting.
• Maintain required health services documentation, including state-specific assessment tools, service plans, progress notes, and resident records.
• Coordinate service plan updates, provider recommendations, medication revisions, nursing assessments, and other changes in condition.
• Manage care coordination with outside healthcare providers, including admissions, care plans, appointments, documentation, and ongoing communication.
• Maintain regular communication with residents’ families and facilitate family meetings to ensure each resident’s needs, preferences, and goals are being met.
• Communicate residents’ dietary needs, allergies, exceptions, and preferences to the appropriate team members, and keep information current.
• Lead ongoing Health Services training, track required staff credentials and testing, and present monthly wellness education for residents.
• Participate in resident retention efforts, scheduled community meetings, and the rotating Manager on Duty schedule, including occasional after-hours responsibilities.
• Perform additional duties as assigned to support residents, team members, and community operations.

Qualifications

• Have or obtain state certifications and licenses.
• Demonstrated experience supervising team members and providing training and support.
• Understand and follow State regulations as well company policies and guidelines.
• English language required.
• CPR Certification (preferred).
• Must pass criminal background check and drug test.
