Job summary

Qualifications

Responsibilities

• Plan and conduct individual, small group, and large group activities.
• Assist in developing and maintaining monthly activity calendars and attendance records.
• Coordinate transportation for residents as needed for outings or appointments.
• Assist in documenting assessments, progress notes, and action plans for quality improvement.
• Support discharge planning and activity care plan development as requested by the Activity Director.
