Job summary

PACS is seeking an Activity Assistant to support the Activity Director in creating a creative and engaging activities program for residents in accordance with applicable standards and guidelines.

The role involves planning and conducting activities, maintaining communication with staff, residents and families, and helping organize monthly calendars and attendance records. It also includes arranging transportation and supporting assessments and care plans as needed.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.

Responsibilities

• Participate in planning and conducting individual, small and large group activities.
• Assist in communication between staff, residents, families and public to meet residents' needs.
• Assist in development of monthly activity calendar and attendance records.
• Assist in discharge planning and activity care plans.
• Arrange transportation for residents to outings, appointments, or discharge.
• Develop monthly activity schedule including outings, group activities, and in-room activities for bed-bound residents.
