Job summary

You will help develop monthly calendars, assist with attendance, arrange transportation as needed, and document progress notes, while maintaining a safe, clean, and welcoming

Qualifications

• High school diploma or equivalent.
• Prefer at least one year of long-term care experience.
• Ability to read and comprehend policy and procedures.
• Ability to communicate effectively with residents, families, and staff.

Responsibilities

• Plan and conduct individual, small and group activities.
• Assist in communication between staff, residents, families, and public.
• Develop monthly activity calendar and track attendance.
• Assist with discharge planning and activity care plans.
• Arrange transportation for residents as needed.
• Assist with documentation and progress notes.
