Job summary

The Pavilion at Piketon is a skilled nursing and rehabilitation center located in Piketon, OH. The Activities Assistant supports planning, organizing, and evaluating recreational and social programs to enhance residents' well-being according to the care plan and supervision.

Key responsibilities include creating accessible activity calendars, interviewing residents for preferences, coordinating transportation, documenting attendance, and reporting changes to the Activities Director.

Qualifications

• High school diploma or equivalent.
• Must be able to successfully pass background check.

Responsibilities

• Plan and post a monthly calendar of activities in large print in a prominent location.
• Ensure the calendar is placed in each resident's room.
• Interview residents or family members to obtain activity preferences.
• Coordinate and assist residents in getting to activity locations within the facility.
• Assist in conducting scheduled activities to promote physical, social, and mental well-being.
• Document resident activity attendance and ensure progress notes are descriptive and informative.
• Observe resident mood and involvement and report changes to the Activities Director.
• Involve residents and families in planning activities when possible.
• Assist with in-room activities for residents who benefit from one-on-one visits.
• Coordinate offsite field trips and transportation when necessary.
• Coordinate activity needs with other departments to avoid interruptions.
• Assist with Resident/Family Council meetings as indicated.
• Other duties as assigned.
