Job summary

The Pavilion at Piketon for Nursing and Rehabilitation is seeking an Activities Assistant to plan, organize, implement and evaluate recreational and social programs for residents in Piketon, OH. The role involves creating monthly activity calendars, coordinating transportation, and ensuring activities align with each resident's care plan.

The ideal candidate will have a high school diploma or equivalent and pass a background check.

Qualifications

• High school diploma or equivalent.
• Must be able to successfully pass background check.

Responsibilities

• Assist in planning, organizing, implementing, and evaluating recreational, social, intellectual, emotional and spiritual programs per facility policy and resident care plans.
• Create a monthly calendar of activities and post it in a prominent location visible to residents and visitors.
• Interview residents or family members to obtain activity preferences.
• Coordinate and assist residents to activity locations within the facility.
• Assist with conducting activities to promote residents' physical, social, and mental well-being.
• Document activity participation and chart progress notes indicating resident response to services.
• Observe resident mood and engagement and report changes to the Activities Director.
• Involve residents and families in planning activities when possible.
• Support one-on-one in-room activities for residents who benefit from it.
• Coordinate offsite field trips and transportation as needed.
• Coordinate activity needs with other departments to ensure daily activities run smoothly.
• Assist with Resident/Family Council meetings as indicated.
• Other duties as assigned.
