CareSource is seeking an Integrated Care Clinical Manager - BH in Massachusetts to monitor, coordinate, and advance clinical care for dually eligible enrollees with complex medical, behavioral, and social needs. The role emphasizes home visits, care plan development, and collaboration with providers and community resources to improve outcomes.

The position requires a Master's degree in social work or mental health counseling with an independent license.

Qualifications

• Master's degree in social work or mental health counseling and independent license required.
• BH Clinician with ability to independently serve people with complex medical, behavioral, and social needs.

Responsabilités

• Engage enrollees in their homes and community settings to establish a complex care management relationship.
• Lead interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Coordinate with healthcare providers, community resources, and state agencies to prevent duplication of services.
• Oversee enrollee utilization of long-term services and supports to allow choice of living location.
• Follow up with members after hospitalizations to ensure continuity of care and prevent readmissions.
