Job summary

CareSource in Massachusetts is seeking an Integrated Care Clinical Manager - BH to monitor and manage care for dually eligible enrollees with complex medical, behavioral and social needs, coordinating health services and community resources to improve outcomes and quality of life.

The role travels across locations, requires an LCSW/LISCW or LMHC license, a Master's degree in social work or mental health counseling, and experience in care coordination.

Qualifications

• Master's degree in social work or mental health counseling and independent license required.
• BH Clinician with the ability to independently serve people with complex medical, behavioral, and social needs.
• Experience in care coordination/case management with dual-eligible populations preferred.
• Medicaid and/or Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in homes and community settings to establish a complex care management relationship.
• Function as a liaison between healthcare providers, community resources and enrollees.
• Lead the interdisciplinary care team and develop holistic care plans.
• Coordinate with community organizations and state agencies to avoid duplication of services.
• Advocate for enrollees' needs within the healthcare system and monitor satisfaction.
