Job summary

CareSource in Massachusetts is seeking an Integrated Care Clinical Manager - BH to lead care management for dual-eligible enrollees with complex medical, behavioral, and social needs. The role focuses on integrating health services with community resources to coordinate enrollee care and improve outcomes.

The incumbent will supervise the interdisciplinary care team, conduct comprehensive and functional assessments, and ensure seamless care transitions while promoting culturally competent,

Qualifications

Master's degree in social work or mental health counseling (required).
Independent license required (LCSW/LISCW/LMHC).
Experience in care coordination or case management with dual-eligible populations preferred.

Responsibilities

Engage enrollees in homes and community settings to establish complex care management relationships.
Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
Coordinate with providers and community resources to ensure seamless care transitions and avoid duplication of services.
Follow up after hospitalizations or significant events to ensure continuity of care and prevent readmissions.
