Job summary

Covenant Case Management Services seeks a Qualified Professional to support individuals with I/DD in a hybrid role in the Charlotte area. You will lead staffing, manage a 25–30 caseload, supervise DSPs, and ensure regulatory compliance across DHHS, Medicaid, and MCO requirements.

You will conduct comprehensive assessments, develop ISP-driven care plans, review documentation, and monitor utilization. Collaboration with QM, Nursing, and SCS teams is essential for high-quality services.

Qualifications

• Hold a valid license, provisional license, certificate, registration, or permit issued by a governing board regulating a human service profession
• Graduate degree in a human service field with at least one year of post-graduate experience
• Bachelor's degree in a human service field with at least two years post-graduate experience
• Bachelor's degree in any non-human service field with at least four years post-graduate experience

Responsibilities

• Staffing and team recruitment to cover caseloads
• Manage caseload of 25–30 active cases with matching of staff to cases
• Lead DSP supervision meetings and ongoing staff training
• Maintain regulatory compliance with DHHS/state rules, Medicaid/state waivers, MCO requirements
• Develop and review ISPs and care plans with measurable goals
• Review and sign ISP documentation and coordinate with Medical Records
• Monitor utilization and correct documentation across staff
• Prompt incident reporting and support IR/IRIS entries for incidents
• Partner with QM on risk mitigation and Plans of Correction
• Lead internal audits and CARF activities with QM and Clinical Directors
• Monitor data and billing accuracy; manage authorizations and service limits
• Engage with clients and stakeholders; secure consents and represent CCMS
• Train staff on client needs and ISP requirements; drive policy compliance
• Remain on-call for crisis services as needed
• Collaborate with SCS Director and Nursing for service updates and meds tracking
• Support leadership with staff development and special projects
• Complete additional operational duties as requested by leadership
